Use of Your Personal Information

This website may include web pages that give you the opportunity to provide us with personal information about yourself. You do not have to provide this information if you do not want to as it is completely voluntary. However, not providing information may limit your ability to use certain functions of the website or to request certain services or information.

Any personally identifiable information you provide to CHRISTUS Health Plan will be used only as permitted by law, including to provide the product, service, or information you have requested or to support the programs for which you registered. Information that you voluntarily provided will be maintained for as long as CHRISTUS Health Plan deems appropriate to fulfill the purpose for which you provided the information, until you request us to remove it, or for as long as permitted under applicable law.

To the fullest extent permitted by applicable law, CHRISTUS Health Plan may also disclose your information if we believe in good faith that doing so is necessary or appropriate to enforce or comply with legal and regulatory obligations (e.g., pursuant to law enforcement inquiries, subpoenas or court orders).
