The information contained in the Provider Directory may change at any time, based on doctor and facility contracts and credentialing activities. There may be a delay in communication between CHRISTUS Health Plan and doctors/facilities, which may affect the information in the Provider Directory. The information contained in the Provider Directory is updated at least monthly. Service may be interrupted due to system maintenance, upgrades, or unplanned outages.
Please check with your doctor or other health care provider before scheduling an appointment to make sure that they are still participating in the network, or call CHRISTUS Health Plan’s Customer Service at the toll-free number on your ID card to make sure your doctor is still in the network.
The Marketplace health plans are designed to have all needed doctors and facilities in the network. This includes primary care doctors, specialty care doctors, and hospitals, skilled nursing facilities, and other types of providers. Specialists and facilities are included if they meet the credentialing requirements and to make sure that members have access to needed doctors within a reasonable distance. Members may request services from doctors who are not in the network, if they can show that there is not a needed type of doctor within 75 miles of the member. Please call CHRISTUS Health Plan’s Customer Service at the toll-free number on your ID card if you have questions about your doctors or need access to a specific type of specialist. Requests will be reviewed on an individual basis.
CHRISTUS Health Plan does not use quality measures, member experience measures, or cost-related measures to select doctors, facilities, or other health care providers for participation in the network.
