The Medicaid Transportation Program (MTP) can provide transportation at no cost to and from scheduled appointments.

As soon as you know your appointment date, call 1-877-633-8747 (Monday through Friday, 8 a.m. to 5 p.m.) to schedule a ride. MTP requires notice at least two work days before your appointment. If you need to cancel, call no later than one work day before your doctor’s appointment.

If a friend or family member is driving you to an appointment, MTP can also help with gas money. Call MTP directly to find out how at 1-877-633-8747.

If you have a complaint or other questions, feel free to call CHRISTUS Health Plan member services at 1-877-428-3057 (1-800-735-2989 for deaf or hard of hearing members).

Care managers are nurses or social workers who can work with your primary care provider to help you learn about your health care, plan appointments or answer questions.

Care managers can also help set up transportation to appointments, get special medical equipment, provide prenatal support for pregnant women or help arrange special help for children who need it due to behavioral, medical or educational needs.

If you or your child needs special care, or you’re interested in our prenatal care management program, call 1-877-428-3057 (1-800-735-2989 for hard of hearing members).

Disease management programs are offered at no cost and help with chronic conditions like asthma, diabetes and more. These programs help you stay healthy by pairing you with doctors and nurses to come up with a care plan that’s right for you. To learn more, call 1-877-428-3057 (1-800-735-2989 for hard of hearing members).

Medicaid offered through CHRISTUS Health Plan does not cover faith healing, acupuncture, cosmetic surgery or any service that is not medically necessary.
