CHRISTUS Health Plan Response to Hurricane Delta

As Hurricane Delta approaches the Gulf coast the governor of Louisiana has declared a State of Emergency. CHRISTUS Health d/b/a Uniformed Services Family Health Plan (USFHP) and CHRISTUS Health Plan want to make sure all members in Louisiana are taking all necessary precautions in advance of the hurricane.  
 
If you need us, call us. USFHP CHRISTUS Health and CHRISTUS Health Plan member services remains open to answer any of your questions and/or concerns.  Please refer to the member services number on the back of you identification card.  In the event that your local physician’s office is closed during this time, there are CHRISTUS caregivers available and ready to serve you. Call  844.CPG.DOCS . Your need will be reviewed by a physician, and someone will follow up with you in 24 hours. Telemedicine visits can also be scheduled. 
 
USFHP members please note that MAXOR pharmacies are expected to be open.  If there is a need to evacuate, please take your original prescription bottle(s) with you and call MaxorPlus (1-800-687-0707) if you need medications. 

For CHRISTUS Health Plans members, pharmacies in the ESI network have procedures for members if a prescription is lost or damaged in the disaster area. The toll-free pharmacy telephone number is listed on the back of you CHRISTUS Health Plan identification card.
